State Police in Huntingdon County say that 70-year old Sally Fultz was living in a personal care facility in the area.
The owner of the care facility, 62 year old Connie Souders is currently being charged with Neglect of Care-Dependent Person and Involuntary Manslaughter.
Souders is the owner and operator of the Sleepy Hollow Personal Care facility/home in Blairs Mills.
Huntingdon County District Attorney George Zanic said Souders was making about $1,300 a month for taking care of Fultz.
"Apparently she was not doing her job." Zanic said. "I keep using the phrase, high degree of negligence."
Zanic added that emergency buzzers on Fultz's doors had been removed by Sauders, creating an unsafe situation.
In 2006, a similar incident occurred under Souders' watch when a 60-year old left the home at night, only to be found in a stream nearby.
Souders has been arraigned and is slated for a preliminary hearing October 30th.
She is currently at the Huntingdon County Jail facing $100,000 bail.
